Can I ask for your kindly help to check if any issue with the install server of powerVM setup?
I also filed a bug to track the issue.

However, I think the issue should have something to do the the test env since it can't be reproduced stably.

I filed a new infra ticket to track the issue as well
Since we hit the issue many times in past 2-3 days.
