Proper fields for the delivery address (and phone)

Description
Add meaningful fields for the delivery address and a contact phone instead of the current generic textarea.
- Status changed from New to In Progress
- % Done changed from 0 to 50
Created a separate reusable model for postal addresses. Tests still need to be adjusted to the new form.
- Status changed from In Progress to Closed
- % Done changed from 50 to 100
Tests adapted and new version deployed in staging.
