Add all needed fields to reimbursement form

Description
- Attachments needs UI enhance and a more robust approach.
- Bank information
- Reports (mostly links but also attachments)
- ...
- Target version changed from First version to Reimbursement polishment
- % Done changed from 0 to 30
- Status changed from New to In Progress
A lot of time invested improving attachments interface.
carrierwave + cocoon + bootstrap-fileupload + nested_attributes can be a dangerous combination in some very tricky corner cases, but everything is working fine now.

Bank information added. With tests. It took longer than expected (as usual).
Only the links are missing now.

Links added. All fields are there now.
